Submit Speaker Forms

Please complete the speaker form prior to March 18, 2019. This will ensure that continuing education considerations can occur and that your photo and biography are included in the printed program and on the Help & Hope WV website. Please complete a form for each speaker and/or session.

• Submit Speaker Form (with photo) by March 15, 2019.
• Community Access will reserve your lodging at the Flatwoods Days Inn & Suites. Please indicate lodging needs on the speaker form.
• Presenters should leverage the conference for professional development as well as to network and build relationships. Be sure to register for the conference by March 15, 2019.
• All PowerPoint presentations must be submitted by April 5, 2019. In order to reduce the volume of paper products used at this training, we are not printing handouts for sessions. Presentations will be available for download and printing from the Help & Hope WV website.
